Thanks for volunteering to serve with us at the North Coast Church Rancho Bernardo Campus! The next step in the volunteer application process is to complete a Live Scan fingerprinting session for a Safety Analysis check, which includes a background check.

- Present this pre-approved Certifix Live Scan flyer to the Live Scan vendor. The flyer will ensure there is no cost to you and allow the vendor to bill us.
- You will also need to present a U.S. government issued identification (driver’s license) and this BCIA 8016 form. Print and fill out your information within the “Applicant Information” located within the yellow box. Make sure the Live Scan operator completes a "Level of Service" for both DOJ and FBI. These should already be marked with an "X" near the bottom section of the form. Note that the waiver includes the Fair Credit Reporting Act summary of consumer rights starting on page 3, but you will see on page 4, we do not check your credit or review any financial information.
- You’ll leave the first page of BCIA 8016 form with the Live Scan business.
- Download the Waiver for Volunteer Safety Analysis Check form and print it out.
- Print your name at the top of the waiver form and sign pages 2 and 6. The waiver includes the Fair Credit Reporting Act summary of consumer rights starting on page 3, but you will see on page 4 we do not check your credit or review any financial information.
- Scan and email all six pages of the waiver to rtubbs@northcoastchurch.com.
Once these steps are complete, your file is sent to our background check company. When the Safety Analysis is complete, we will update your profile in our church database indicating that you have been cleared to volunteer to serve with North Coast Church.
