This is a big step in your faith journey, and we’re so excited to walk alongside you. Before you fill out this form, here are a few things to know to help prepare you for what’s next and to be able to complete this form.

On this page, you’ll find a video that explains what baptism is and how it works. If you haven't already done so, please watch it before you continue—it’s a key part of understanding what this moment means.

• Baptism Date – Go ahead and set a date that works for you and include it in the form so we can follow up and celebrate with you.
• Who Will Baptize You? – If you already have someone in mind, that’s great! You’ll enter their name in the form so we know who you chose to baptize you. If you're still deciding who should baptize you, please take a moment to read the following How to Choose Someone to Baptize You section below for what we believe about making this important choice.
• What’s Your Story? - Baptism is a public declaration of your decision to follow Jesus. We’ll ask you to briefly share your story—what led you to surrender your life to Christ. It doesn’t have to be long, just honest and from the heart.

How to Choose Someone to Baptize You
Baptism is a public declaration of your faith in Jesus, and choosing someone to baptize you is a meaningful part of that. Here are a few key things to consider:

1. They are a follower of Jesus.
   • They believe Jesus is their Lord and Savior and are committed to living out their faith.
2. They model a life of faith you respect.
   • Their character, love for others, and devotion to Christ are things you admire and want to emulate.
3. They have been baptized themselves.
   • Baptism is both a symbol and a step of obedience—so it’s meaningful when the person baptizing you has taken that same step.
4. They’ve played a role in your spiritual journey. (Suggested)
   • This could be a parent, mentor, pastor, friend, or small group leader, someone who has walked with you in your faith.

Don't Have Someone to Baptize You?
Choosing someone can be a hard choice. Or maybe it would be an easy choice for you, but that person isn't available on your selected date. Either way, if you don't have someone selected, we'll partner with you to figure out who will baptize you!

• You’ll receive a confirmation email with all the details of what you submitted.
• We'll be reaching out to confirm the details and next steps for your baptism.
• If you have selected someone to baptize you, please make sure that person is confirmed and available to baptize you on your selected date.
• If you don't have someone selected to baptize you, don't worry, we'll partner with you to figure out who will baptize you.
• Once we've been able to connect with you about your baptism and confirm you are all set to go, we’ll order and send your “Made New” T-Shirt for the big day. If you are in an area near one of our campuses, you will have the option to pick it up in-person instead.
• After your baptism, we’ll follow up to confirm it happened as planned. We'll also ask for you to share any photos and anything exciting that happened during your baptism so we can celebrate with you! Some of your photos may even be featured in a future video! After that we'll send your digital baptism certificate.
